Admission Criteria and Facility Tours for Three Links Care Centre

If you think you or a family member needs to move to a residential care facility, your first step is to call the Community Health Centre in the area of the city where you live. In Vancouver, there are 8 centres to contact:

A case manager will work with you and your family to determine your care needs and to review your options. The level of care you require will be determined by an assessment of both physical and mental capabilities. If you are in hospital, a social worker will assist you in completing an assessment.

If you or your family member has been assessed for Long Term Care and would like to tour Three Links Care Centre, please call our Social Worker at 778-452-6504 or send an email. Please provide us with your name, phone number, the level of care needed and a few dates and times when you would be available for a tour. Alternatively, you can also request a tour online.

Please allow for at least two days to receive a response.

Residency Inquiries for Three Links Manor, Three Links Houses and Odd Fellows Manor

To apply for residency at Three Links Manor (Supportive Housing), please complete an application form.

To book a suite at Three Links Houses, please complete an application form. All suites are booked on a first come, first served basis, and require a minimum of four days’ advance notice.

To apply for residency at Odd Fellows Manor, please complete an application form.

All completed Housing Application Forms can be submitted online or faxed to 604-568-7499, attention Housing Services. For more information, please contact Housing Services at 604-568-6895 or send an email.

***PLEASE NOTE:  If you are using a public computer (library, internet cafe, etc.) to send an online application form, the online feature will not work, as a personal email account is required.  In this case, you must print your appplication form and fax or mail it to Three Links Care Society.***

Applications for Three Links Manor, Three Links Houses and Odd Fellows Manor may also be dropped off or mailed to:
